Purpose: This study identifies cognitive biases and blind spots in internal safety inspections of mid-sized construction companies to improve autonomous safety management systems. Method: Using 2-year ('2023–2024) empirical data from a mid-sized construction company (Company A)—comprising 2,119 internal inspection findings and 80 actual accident cases—frequency analysis, cross-tabulation, Gap Index (Gi), and Disparity Ratio (Ri) were derived, followed by a Chi-square goodness-of-fit test. Result: The Chi-square test revealed a statistically significant discrepancy between inspection findings and actual accident distributions(X2=176.302, p<.001). While inspections were heavily concentrated on ‘falls’(35.7%), actual accidents were predominantly ‘slips/trips’(35.0%) and ‘collisions/jamming’(27.5%), proving that dynamic work environments remain critical management blind spots. Conclusion: Departing from static, temporary structure-centered inspections driven by visual accessibility, it is essential to redesign dynamic checklists linked to risk assessments, activate TBMs (Tool Box Meetings), and reallocate safety resources based on actual accident risks.
