Purpose: At the scene of a mass casualty incident, rapid and accurate triage is required despite limited resources. However, the paper-based triage tags currently in widespread use suffer from limitations such as poor visibility in low-light or smoke-filled environments, information loss due to contamination or damage, and time delays caused by manual entry. To address these limitations, this study aims to develop a low-cost, Arduino Uno-based smart triage device incorporating the START(Simple Triage and Rapid Treatment) algorithm and to evaluate its field applicability through simulations. Method: This study is a simulation-based randomized case-control study. Ten participants-university students majoring in non-health-related fields or office workers-received one hour of START triage training on the day of the experiment. They were then presented with five patient scenarios(two Red, one Yellow, one Green, and one Black), and the time required for triage and information entry was measured for each participant using two methods: paper tags(control group) and smart devices(experimental group). The smart device was constructed using eight physical buttons operable while wearing gloves, an I2C LCD, and an HC-06 Bluetooth module. Data were analyzed using SPSS version 30.0, and the Mann-Whitney U-test was performed to compare the time required between the two groups. Result: The mean input time for the case group was 2.34±0.45 seconds, which was significantly shorter than that of the control group(mean 3.89±0.71 seconds)(Mann-Whitney U=0.0, p =.008, rank-biserial r=1.00). No data loss or wireless transmission errors were observed during the repeated and continuous input processes. Conclusion: It was confirmed that the Arduino-based terminal for smart triage and wireless patient data transmission significantly reduces the time required for triage and data entry compared to paper tags. However, as this study was a pilot feasibility study involving very small, independent groups, the findings cannot be generalized as clinical or operational evidence; further field-based validation studies involving larger, independent samples, actual disaster environments, and emergency response personnel are required.
